Young people with ASD who are receiving care from CAMHS or a child health team should be reassessed at age 14 to establish whether they will need an adult service.
If an adult service will be required, give information to the young person about services and treatment that will be needed.
Prepare for a smooth transition, which should be completed by the age of 18.

Consider a formal meeting involving health and social care and other relevant professionals from child and adult services.
