“A health passport is a document that provides professionals with important information needed when caring for someone with a learning disability, long term health condition, mental health issues, or who is autistic whilst in hospital.

The document includes a traffic light system; Red– things you must know about me, Amber– things that are important to me, Green– My likes and dislikes.
